Enrolling is super easy and it's good to do early because it ensures you are funded before your appointment - giving you access to affordable healthcare under the New Zealand public health system.
Follow the steps below to complete your enrolment.

Prefer to enrol in person? Visit our clinic to complete a physical enrolment form or print the enrolment and healthcare forms at the top of this page and bring these to the clinic with you .

Identification Requirements
To enrol, you’ll need to provide proof of your eligibility to access publicly funded healthcare in New Zealand.
Please ensure you bring:
1. Primary Identification (One of the following):

A New Zealand Passport
-
A New Zealand Birth Certificate (includes Cook Islands, Niue, or Tokelau birth certificates)

A New Zealand Certificate of Citizenship

A Descent Registration Certificate
-
Evidence of receiving a social security benefit (excluding emergency benefits)
2. Supporting Documents (Two forms, including one with a photo):

A driver's licence, student ID, or other photo ID
-
A recent utility bill, bank statement, or other document that confirms your address
Eligibility for Enrolment
To be eligible for enrolment at our clinic, you must reside in New Zealand permanently* and meet one of the following criteria:

New Zealand Citizens
-
Includes those born in New Zealand.

Resident or Permanent Resident Visa Holders
-
Includes residence permits issued before December 2010.
-

Australian Citizens or Permanent Residents
-
Must have been in New Zealand or intend to stay for at least 2 consecutive years.

Work Visa Holders
-
Must show eligibility to stay in New Zealand for at least 2 years (including previous permits).

Interim Visa Holders
-
Eligible if immediately eligible before the interim visa started.

Refugees, Protected Persons, or Victims of Trafficking
-
Includes those applying for or appealing refugee or protection status.

Dependants Under 18 Years Old
-
Must be in the care of a parent/legal guardian who meets one of the above criteria.

18–19-Year-Old Dependants
-
Eligible if dependent on a valid work visa holder as of 15 April 2011.

Special International Students or Scholars
-
Includes:

NZ Aid Programme students receiving Official Development Assistance.

Participants in the Ministry of Education Foreign Language Teaching Assistantship scheme.

Commonwealth Scholarship holders funded under the Commonwealth Scholarship and Fellowship Fund.

*“Permanently residing” means intending to live in New Zealand for at least 183 days in the next 12 months.

Enrolment Duration
-
Enrolment lasts up to three years without a consultation, updated enrolment form, or verified contact confirming ongoing enrolment.

Best practice involves re-confirming enrolment via a signed form or auditable contact (e.g., documented phone calls, not text messages).

After three years without contact or a consultation, patients will be removed from the Enrolment Register.
Dis-enrolment and Termination

Patients can dis-enrol at any time.
-
Enrolment may be terminated if:
-
The relationship with the practitioner is compromised.
-
Records show the provider is no longer the patient’s regular healthcare provider.
-
Patients will be notified of termination and supported in finding a new provider if needed.

Transfer of Information
-
Patient information is managed under the Privacy Act 1993 and related codes.
-
Information may be shared with health professionals directly involved in patient care, ensuring confidentiality.
Provider and PHO Changes
-
Changes in providers or PHOs are communicated to patients at their next point of contact or through general practice advertising.
-
If a provider changes affiliation, the enrolment date remains unchanged, and the three-year enrolment period continues uninterrupted.
Patient Responsibilities

Patients must provide accurate and truthful information during enrolment.

Updates to personal information must be communicated promptly to ensure continuity of care.

How can I book an appointment?You can book an appointment online through our website, via the patient portal, or by calling us directly.

Can I access my test results online?Yes! Enrolled patients can access test results through the patient portal (Manage My Health) once reviewed by a doctor.

How do I update my contact details?You can update your details online via the patient portal, by calling or visiting the clinic.

Do you accept walk-in patients?We recommend booking an appointment to ensure availability. However, we do accommodate walk-ins for urgent cases, subject to doctor availability.

What are your opening hours?We're opening Monday to Friday - 8:30am to 4:30pm.

Why do I need to provide identification?Identification is required to confirm your eligibility for publicly funded healthcare in New Zealand

Can I enrol my family members?Yes! Please note that each family member needs to complete their own enrolment form and provide their identification documents.

What if I’m not eligible for publicly funded healthcare?If you’re not eligible, you can still see a doctor at our clinic, but these will be with our non-funded fees. If your insured we will give you an invoice you can use to make a claim through your insurance provider . Contact us in person or by phone for more information.

Do I need to re-enrol every year?No, but you must update us if your personal or contact details change. If you haven't been seen in the last three years we may contact you to confirm enrolment.
